How to get a Indiana occupational therapy license
Occupational therapists in Indiana are licensed by the Indiana Occupational Therapy Committee. Below is what the board requires, from education to renewal, with every requirement linked to the board page it comes from. Checked October 22, 2026.
Step by step
Getting licensed
- Education
- Applicants must submit a transcript from the school where the occupational therapist obtained the degree showing the degree conferred; graduates of foreign programs must submit an official translation if the transcript is not in English. [1]
- National exam
- Applicants by examination must have their NBCOT exam score report sent directly to the Indiana Occupational Therapy Committee. [1]
- Background check
- Applicants for an initial license must submit to a national fingerprint-based criminal history background check at their own cost (listed as $38.20), after the license application has been submitted; occupational therapists are among the license types that require it. [2]
- Fees
- The occupational therapist application fee (by examination or by reciprocity) is $100.00, paid by credit or debit card and nonrefundable. [1]
- Where to apply
- Applications are submitted online through the Indiana Professional Licensing Agency's online licensing services, linked from the committee's licensing information page. [1] Open the application →
Moving to the state
Already licensed somewhere else?
Indiana is a OT Compact member and is issuing compact privileges: occupational therapists and occupational therapy assistants licensed in another member state can obtain a privilege to practice in Indiana without applying for a new state license.
- Licensed in another state
- OTs licensed in another state may apply by reciprocity with the $100.00 fee, a criminal background check, and license verification sent from the issuing state; at least 1 verification is needed to issue the license, and any remaining verifications are due by the first Indiana renewal. [1]
- Interstate compact
- Indiana adopted the OT Compact under Senate Enrolled Act 73-2023, and eligible OTs and OTAs may now apply for compact privileges directly through the OT Compact. [1]
Before your full license
New graduates and clinical fellows
- Temporary or provisional license
- After applying for full licensure, applicants may obtain a temporary permit ($50.00) to practice only under the direct supervision of an Indiana occupational therapist; it expires at the earliest of permanent licensure, disapproval of the application, or 180 days after issuance. [1]
After you are licensed
Keeping the license active
- Renewal
- Occupational therapist licenses expire December 31 of even-numbered years; the renewal fee is $100. [1]
- Continuing education
- OTs must complete at least 18 hours of continuing competency in each 2-year license period, at least 9 of them in Category I courses, with no more than 9 hours from Category II activities. [1]
